Service Description
Comprehensive physical exams for personal, school, employment, and healthcare program requirements.
This visit includes a review of your medical history, vital signs, and a head-to-toe physical exam. Required forms will be completed during your visit.
Common physicals include:
• Personal/ Nursing / Healthcare Program Physicals
• School Physicals
• Pre-employment Physicals
Please bring all required forms and immunization records if available.
Additional services available if needed:
• TB testing (PPD skin test or blood test)
• Immunization review or titers
• Lab orders
TB testing, labs, and immunizations are not included in the base visit and may require a follow-up visit.
Note: Final clearance is based on clinical evaluation and provider discretion .
Cancellation Policy
Appointments may be rescheduled with at least 24 hours’ notice. Cancellations made within 24 hours of the scheduled appointment will incur a 50% late cancellation fee. Missed appointments without notice are subject to a 100% no-show fee. Payment is required at the time of booking to reserve your appointment.
