Service Description
Follow-up visits provide ongoing medical weight management support, medication monitoring, and adjustment of your treatment plan as needed. This visit allows us to review progress, discuss side effects, and ensure your plan continues to support your long-term health goals.
Cancellation Policy
Weight Management Program Policy Be Seen, LLC provides medically supervised weight management services led by a licensed Nurse Practitioner. Program Structure Weight management services are delivered in structured phases: • Initial medical consultation • Treatment initiation (if clinically appropriate) • Ongoing monthly follow-up visits Continued participation requires active follow-up and compliance with recommended monitoring. Medical Eligibility All treatment recommendations are based on clinical evaluation. Medication approval is not guaranteed. Certain medical conditions, laboratory findings, or medication interactions may delay or prevent treatment initiation. Labs & Monitoring Some patients may require laboratory testing prior to starting medication and periodically during treatment. Patients are responsible for completing required labs in a timely manner. Medication Policy Medications are prescribed only when medically appropriate. Refills require active follow-up visits. Missed appointments may delay refills. Payments & Fees Consultation and follow-up fees are due at the time of booking. Medication costs, lab fees, and pharmacy charges are separate and may vary. Cancellations Appointments require at least 24 hours’ notice to reschedule. Missed appointments are non-refundable. By enrolling in the program, you acknowledge and agree to these terms.
