Service Description
Comprehensive physical exams for school, employment, and healthcare program requirements. This visit includes a review of your medical history, vital signs, and a head-to-toe physical exam. Required forms will be completed during your visit. Common physicals include: • Nursing / Healthcare Program Physicals • School Physicals • Pre-employment Physicals Please bring all required forms and immunization records if available. Additional services available if needed: • TB testing (PPD skin test or blood test) • Immunization review or titers • Lab orders TB testing, labs, and immunizations are not included in the base visit and may require a follow-up visit. Note: Final clearance is based on clinical evaluation and provider discretion.
Cancellation Policy
Appointments may be rescheduled with at least 24 hours’ notice. Cancellations made within 24 hours of the scheduled appointment will incur a 50% late cancellation fee. Missed appointments without notice are subject to a 100% no-show fee. Payment is required at the time of booking to reserve your appointment.
