Service Description
If you have Medicare Part B, you’re entitled to a free Annual Wellness Visit every 12 months. This is NOT a sick visit — it’s a dedicated appointment focused entirely on your health history, preventive screenings, and building a personalized plan to keep you well. What’s included: • Review of your medical and family history • Update of your medications and health risk factors • Blood pressure, height, weight, and BMI • Cognitive assessment • Personalized prevention plan • Referrals for recommended screenings (mammogram, colonoscopy, bone density) $0 out of pocket for most Medicare patients. Cancellation Policy
Appointments may be rescheduled with at least 24 hours’ notice. Cancellations made within 24 hours of the scheduled appointment will incur a 50% late cancellation fee. Missed appointments without notice are subject to a 100% no-show fee. Payment is required at the time of booking to reserve your appointment.
